New Patient Forms

Every new patient visit starts with the same short list of details: your health history, your medications, your insurance information, and what brought you in. Handling that digitally means you are not writing it out on arrival. We send the link by text or email, and you finish it whenever the timing works for you.

We ask for completed forms 48 hours before your appointment so we can verify coverage, prepare your chart, and have your room ready before you get here. Patients at our Fortville and Westfield office usually go from the front door to the treatment chair in just a few minutes.
